Conquering Colon Cancer in Greater Franklin County

This proposal is to further expand Healthy Community Coalition's (HCC) proven effective work reaching out to economically disenfranchised, rural residents of Greater Franklin County with cancer prevention and early detection education and screening. In response to recent local data,including findings from the 2013 Community Health Needs Assessment, HCC is seeking funding to improve colorectal cancer detection and outcomes. HCC will also provide community education to promote healthier lifestyles as a general cancer prevention strategy beneficial to all.  HCC will use a multi-faceted approach:

1) provider driven patient education

2) public education using local media to implement a community-wide campaign

3) community outreach and screening days at local events and gathering places of the target audience.
